Claiming 8.0 hours on (DATE) I requested a meal enroute at the initial terminal of (LOCATION) at (TIME) on (ASSIGNMENT) from (DISPATCHER NAME or INITIALS). I operated from (LOCATION) to (LOCATION) and I was denied a meal enroute by train dispatcher (NAME or INITIALS). Restaurants and other locations were available enroute for a meal period. Per the dispatcher's instructions, I operated to the final terminal without being provided a time to eat. I was on duty a total of (TOTAL TIME ON DUTY).
