NEED: Date, Assignment, Time on duty, Location, Times meal period observed

Claiming 8.0 hours on (DATE) account was not provided a meal period within the meal bracket. Yard meal periods are to occur between 4 1/2 and 6 hours after starting work. While working yard assignment (ASSIGNMENT) at (LOCATION), I observed a meal period during my tour of duty between (TIME STARTED) and (TIME ENDED). I was on duty (TOTAL TIME ON DUTY).
